Request Materials

Purchase Requests

The Library welcomes recommendations for materials to add to our collection that support curriculum and promote professional development. Use this form to submit your request for consideration. Make sure you include contact information for follow-up from Library staff.

Reserve Videos

Faculty needing to reserve a DVD should complete one form per video at least one week in advance. Access the Video Reserve Form here. Instructors will receive an email confirmation and a reminder email when videos become available one week before the request date. All reserved videos can be picked up at the Fort Wayne library circulation desk. For more information or questions, contact Carol Gibbs at 480-2019 with questions.
